How Home Care Works 

Step 1: Initial Contact

Getting started begins with a phone call or online request. A member of our care team will listen to your needs, answer questions, and explain available service options.

Step 2: Eligibility & Coverage Review

We review insurance coverage and eligibility, including the North Carolina Medicaid Standard Plan when applicable. This step may include benefit verification, authorization requirements, and coordination with managed care organizations or referral sources.

Step 3: Care Assessment

An in-home or virtual assessment is conducted to better understand the individual’s needs, preferences, and daily routines. This assessment helps determine the appropriate level of care and supports a person-centered approach.

Step 4: Care Plan Development

Based on the assessment, a customized care plan is developed. The care plan outlines services, schedules, and support needs while respecting the individual’s goals, dignity, and independence.

Step 5: Caregiver Matching

We thoughtfully match caregivers based on skills, experience, availability, and compatibility. All caregivers are screened, trained, and supported in alignment with state requirements and agency standards.

Step 6: Care Begins

Once services are authorized and scheduled, care begins according to the approved plan. Our team maintains communication with clients and families to ensure continuity and quality of care.

Step 7: Ongoing Monitoring & Support

Care needs can change over time. We regularly review care plans, address concerns, and make adjustments as needed to ensure continued safety, comfort, and satisfaction.
